Connect. Grow. Thrive. offers a variety of services to support any and all individuals with intellectual and developmental disabilities.
Host Home Provider
Connect. Grow. Thrive. provides Individual Residential Services and Supports (IRSS) for up to three people (3rd room has to be approved by the third room waiver) receiving services in a family and host home setting. A variety of living arrangements are established to meet the unique needs for support, guidance, and habitation of each person under our care. We are committed to helping people work towards their dreams and goals while offering high quality services.
CHRP Provider
CHRP is a waiver through the Colorado Medicaid Program for children with developmental disabilities that enables them to pursue various therapeutic activities while remaining in their home and community. The services offered to them include supported community connection, respite, music therapy, speech therapy, occupational therapy, hippotherapy, and the residential program. The residential program is where a parent can choose to place their child in a home where they would live with a caregiver and their family.
Respite Care
Connect. Grow. Thrive. offers short term daily or hourly respite services in the individual’s home or in the community.
Community Connection Program
Connect. Grow. Thrive. helps participants access a wide array of experiences and education in their community life. Types of experiences are including but not limited to typical experiences of people in the general population, such as community leisure, recreation, education, training, and volunteer opportunities.
Non-Medical Transport
Connect. Grow. Thrive. provides transportation services under the DD and SLS waiver, as outlined in the person’s Service Plan (SP), for people who do not have natural supports or other means of acquiring transportation.
